#1 Why does the fire go out with the door closed?

I would guess the tinder was wet, not enough, or the wrong technique. I suggest you watch a video on the web about it. Also if the outside temperatures are -10f or below, you may want to preheat the air in the chimney help positive drafting. I use a propane torch for a few minutes. This prevents filling the room with smoke.

#2 Why wont my 6 inch pipe fit?

1) If you're using a double wall pipe, you will need an adapter.
2) One end of the pipe is crimped; the other end is not. Turn the pipe around.
3) The pipe will only go down about 1/2 inch. Then there is 3 screw holes to keep it in place.

#3 How do you control how much air is let into the heater? Is the flue damper the only control that you have over the flow? Is a damper included?

No flue damper is required. My stove burns from front to back in the middle of the optimum burn temperature range.
